Conversely, the driver's front steering wheel airbag failed to deploy completely during the impact. A corporate inspection by Toyota Motor North America/Engineering Analysis Associates confirmed severe, localized separation of the right front strut, knuckle, tie rod end, brake line, and ABS wiring.
That message is now gone, but airbag and seatbelt lights remain active. I suffered knee injuries from contact with the dashboard, and my child sustained a busted lip and head pain. I’m concerned that a malfunction in the airbag or supplemental restraint system contributed to these injuries.
The contact stated while at a stop sign, the brake pedal was depressed; however, the brakes failed to engage and the vehicle unintentionally accelerated which caused the front driver's side of the vehicle to crash into another vehicle. There was severe cosmetic damage however, no injuries was sustained.
The contact stated that while his father was driving at 35-37 MPH down a hill, he applied the brake pedal, but the brakes failed to respond. The contact continued to pump the brake pedal. The driver moved the gear shifter, attempting to slow down the vehicle.
The contact's wife had no control of the steering wheel. The contact stated that the vehicle made a hard right turn. The contact stated that his wife tried everything to stop the vehicle, however the vehicle was uncontrollable.
While the contact's wife was driving at unknown speeds, the messages stating "vehicle notification system, a malfunction in the vehicle notification system has been detected, contact dealer", and "notification system malfunction visit your dealer" were displayed.
