My wife had properly secured ellie in her seat belt but she was ejected from the auto and died instantly in a low speed crash. She may have unbuckled or herself but our major concern is that the car had previous seat belt failure and we have the maintenance records to support this.

Child in the third row, center was ejected from the vehicle because seat belt failed to restrain him.
Consumer was wearing seat belts, but the air bags did not deploy. Several months later, consumer's son swerved to avoid hitting a deer. Vehicle rolled into a ditch, flipped on its roof, and slid down an embankment. Seat belts were worn, but the air bags failed to deploy. Driver sustained minor injuries .
Put shift to park. Exited vehicle. Vehicle started moving in reverse. Driver's door was open - ripped of by garage. Finally jumped back into vehicle after it went down driveway, across street, & into neighbor's yard, stopped vehicle. Looked at gear indicator - still read park.
The contact's neighbor, an independent mechanic, diagnosed that there was fuel leaking between the connection of the fuel line and fuel tank. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 80,000.
Vehicle did not have an overheating issue, and has been well maintained since I've owned it. Drove across a steep 7 mile hill, temp gauge started moving above normal. I stopped and let it cool down, motor got to about 245 degrees well before any warning lights were able to come on.
